You need to collect a new patient registration pack from our reception in Cudworth or Grimethorpe.
Once you have completed the forms within the pack, please return them to us, bringing ID (to confirm you are who you say you are) and proof of address (to ensure you live within our practice boundary). We will process your request and inform you of the outcome following this. Please ensure you have about a month’s supply of medication from your previous surgery at the time of registering with us to ensure you won’t be without it.
If you are already registered with a practice in the local area, we advise you to remain with your practice. Although we accept patients living within our practice boundary, we prefer not to take on patients already registered locally.
