Annual reviews and what happens:

Many patients have a chronic disease which needs an annual review. If you want to know what to expect during the reviews, please check below. Annual reviews are generally due in the month the patient’s birthday falls. Only January, February and March birthdays fall in April, May and June instead:

Asthma Annual Review:

The nurse will discuss your asthma symptoms and treatment with you, check your inhaler technique and will check if you are still on the best evidence-based treatment for your asthma. Please bring your inhalers with you to the appointment.

Atrial Fibrillation Annual Review:

The nurse, nursing associate, or healthcare assistant will take some blood tests, check your blood pressure and your pulse, and measure your height and weight. As part of your review, we would like you to bring a urine sample in a white-topped bottle to the appointment which we will send to the hospital to check for protein. The nurse, nursing associate, or healthcare assistant will take some blood tests, check your blood pressure and your pulse, and measure your height and weight. Chronic Obstructive Pulmonary Disease (COPD) Annual Review:

The nurse will discuss your asthma symptoms and treatment with you, check your inhaler technique and will check if you are still on the best evidence-based treatment for your COPD. Please bring your inhalers with you to the appointment.

COPD also includes conditions like bronchiectasis, chronic bronchitis, and emphysema found on a lung health check.

Dementia Annual Review:

The doctor, nurse or dementia champion will discuss how you and your loved ones are coping with your diagnosis and discuss any additional help you may require and how to access this. It is aimed at allowing you to live well with dementia.

Diabetes Annual Review:

This review comes in 2 parts, the Year of Care 1 and the Year of Care 2

During the Year of Care 1 appointment, the nurse, nursing associate, or healthcare assistant will take some blood tests, check your blood pressure and your pulse, examine your feet and measure your height and weight. As part of your review, we would like you to bring a urine sample in a white-topped bottle to the appointment which we will send to the hospital to check for protein. During the Year of Care 2 appointment the nurse will discuss your diabetic control with you and whether additional measures are needed to improve this control (advice on diet, lifestyle, or additional or sometimes a reduction in medication). Achieving good diabetic control is important to help avoid or delay any complications from your diabetes.

Diabetes is not an innocent disease and can lead to serious complications, including loss of sight or limbs, heart attacks, kidney failure, and others. The risk of complications is increased in uncontrolled or unmonitored diabetes. Please attend your review when you are invited or know you are due.

Heart Failure Annual Review:

Hypertension Annual Review:

Learning Difficulties Annual Review:

The nurse may take some blood tests depending on other conditions you have, and will check your blood pressure and your pulse, and measure your height and weight. As part of your review, we would like you to bring a urine sample in a white-topped bottle to the appointment which we will send to the hospital to check for protein. Medication Review:

The nurse, nursing associate, or healthcare assistant will take some blood tests, check your blood pressure and your pulse, and measure your height and weight. A second (telephone) appointment is needed with our pharmacist for the second part of your medication review. The pharmacist will phone you during this appointment and discuss your medication and results with you.

Mental Health Annual Review:

People diagnosed with a significant mental health condition are entitled to an annual health check. The nurse will take some blood tests, check your blood pressure and your pulse, and measure your height and weight. Non-diabetic Hyperglycaemia Annual Review:

During this appointment the nurse, nursing associate, or healthcare assistant will take a blood test. This condition is pre-diabetic and an annual blood test will help diagnose diabetes at the earliest opportunity. Peripheral Arterial Disease Annual Review:

Stroke Annual Review:

This review is also for people who have had a mini stroke, or TIA.
