We only accept patients who live within our practice boundary. You can find what our boundary is and check you live within the area by looking at the map below:
Catchment Area
You need to collect a new patient registration pack from our reception in Cudworth or Grimethorpe.
Once you have completed the forms within the pack, please return them to us. Please bring the following with you
- ID (to confirm you are who you say you are)
- proof of address (to ensure you live within our practice boundary).
We will process your request and inform you of the outcome following this.
Non-urgent advice: Before Registering
Please ensure you have about a month’s supply of medication from your previous surgery at the time of registering with us to ensure you won’t be without it.
If you are already registered with a practice in the local area, we advise you to remain with your practice. Although we accept patients living within our practice boundary, we prefer not to take on patients already registered locally.
